Intake Forms
Medical Intake Form
Required for all new primary care patients. Includes family history, current medications, and past surgeries.
Dental Intake Form
Required for all new dental patients. Focuses on oral health history and previous dental work.
Insurance & Billing
Accepted Dental Insurance
We accept most major Canadian dental insurance plans, including Sun Life, Manulife, Great-West Life, and Green Shield. We offer direct billing to your insurance company as a courtesy, meaning you only pay the co-pay or deductible at the time of your visit.
OHIP Coverage
All standard medical visits to our family physicians are fully covered by the Ontario Health Insurance Plan (OHIP). Please ensure you bring a valid health card to every appointment. Uninsured services (such as sick notes or third-party physicals) will be billed directly to the patient.
Cancellation Policy
We require 24 hours notice for all cancellations. Missed appointments or late cancellations will be subject to a $50 fee.
